When ceremorphosis is performed on a svirfneblin, it usually results in the death of the host and tadpole. However, when combined with a dark ritual, a gnome-flayer will be created. These small stealth operatives retain their deep gnome stature, but the unique process of their ceremorphosis mutates their bodies. The mutations are not consistent or uniform among all gnome-flayer, so you'll rarely see two that look alike. Their bodies become malleable, allowing their psionics to reshape their body in almost any way they please."
The Skimlexis is a monster found in our D&D supplement Masters of the Mind: The Book of Illithid. Below is an excerpt from Masters of the Mind:
Gnome-flayer are masters of reconnaissance, able to hide in plain sight by shapeshifting into small creatures, or even non-living objects, sitting in the same room as you without you ever knowing. However, a gnome-flayer may not always want to risk such a bold move, in which case it might merge with shadows, or spy on you through the senses of animals like your pets or steed.
